A Clinical Study of the Burns


Abstract
142 cases of the burn patients experienced at the department of General Surgery, Chu¡þngnam National University Hospital, for 4 years from 1975 to 1978, were analysed.
The results are as follows;
1. The incidence of the bum has been increased every year and the mean incidence was 5.7% of all the admitted patients at the department of General Surgery.
2. The incidence of the adult to the pediatric group was almostly similar. The incidence of The male to the female was almostly similar in the pediatric group, but 1. 7 times more frequent in male.
3. The burn patients were most common in Octber, August and September, and seas¡þonally in Summer(33.6%).
4. The most common cause of burn was the flame burn(57.8%). The other causes
were the scald burn(35.2%), the electrical burn(3.5%), the contact burn(2. 1%), and the chemical burn(1.4%).
5. The precipitating causes were the pediatric¢¥s accident(38. 8%), industrial accident (27.5%), the raw manipulaton of the alcohol burner in the camping(15. 7%), the incide¡þntal accident during sleeping(10. 6%), suicidal behavior(4. 9%), and others(1. 4%) in order.
6. The mortality was 11. 9% (17cases) of all the burn patients, and increased in prop¡þortion to the burn ranget. The causes of death were inadequate fluid therapy and septic¡þemia
7. In the bacteriologic cultures of the burn areas, the positive rate was 58.3% (7 cases),
and among them, Pseudomonas aeruginosa was cultured in 42.9% and Staphylococcus aureus in 28.6%.
8. The frequently involved areas were the arm(60. 5) and the face(59.2%).
9. The most commonly used drug for treatment was 0. 5% AgN03 solution (33. 5%). The effect for the prevention of the infection was best in 0.5¡Æ0 AgN03 solution (infection rate : 67. 8%). 10% Sulfamylon cream was an also an effective drug(infection rate : 75.0,¡Æ0).
10. The common complications of 71 cases, except self-discharge, were local infection (85.9%), joint stiffness(76.1%), renal failure(16. 9%), septicemia(7. 1%), Marjolin¢¥s ulcer(5. 6%), and others(2. 8%).
